  Another summer has passed and time to start thinking about "Fall Clean-up" and winter plans and preparation. As the activities normally associated with summer wind down our morale and good spirits often decrease as well. However, there are others who see the advent of winter i.e., lots of cold, grey, wet days here in the Pacific Northwest, as an excuse for avoiding yard work and delaying other outdoor projects and turning to the writing, reading, and corresponding we've put off for the past few summer months.

  Could be that one of the things you were going to do, when "the heat goes out of the sun" and you found yourself indoors, was to jot down a few things you'd been thinking about that happened to you before, during, and after your Vietnam service. And, maybe you even thought about sending your recollections off to the Foundation to make sure the world doesn't forget what you and your Brothers did in those days - things funny, occasionally sad, more often just every day stuff, or even dangerously heroic, and a few unmentionables that spice up those sea stories. Or, could be you'd planned to call the Foundation and take advantage of the offer of a phonecon interview that would save tons of time in writing down all the stuff you did and leave it to the pros to smooth up.

  Don't lose that thought! Start writing as soon as you find a piece of paper and a pen or when next you turn on your computer. Getting started is often the most difficult part of writing. Barring that, call the Foundation and set up a time for an interview. Don't be shy.

  The Foundation's mission is embodied in our slogan "You made history. Your Foundation is making it known." Please help us to accomplish our mission.

"The Book" SitRep:  October

  First a reminder: The Book staff is developing an annex that features pictures of tanks, Retrievers, Ontos, and trucks that display nicknames e.g., Flame tank tube w/"Crispy Critters"; gun tank tube w/"Ho's Greatest Nightmare"; etc. Presently there are

numerous Names and Logos entries. Please submit the nickname/logo, with pictures if available, of your vehicle and we'll fold them into the annex. Even if you have no pictures, exercise your memory and send us an e-mail with the nickname or "art" work you recall during your time fighting the Vietnam War.

  Also The Book will provide a "Glossary of Terms, Abbreviations, and Acronyms". We're calling for our members to assist the population of this annex as well. The annex will be more than a list ("H&I Fires - Harassing and interdicting fires".) but details, examples, an explanation of what "H&I" meant to the Tanker with a story or two. The "M3 .45 Caliber Submachine Gun - Grease Gun" will be listed in the annex not simply as a weapon of the M48A3 TO/E but with some of the stories we have in the Foundation's archives of how it was used in combat. This is a significant undertaking and one which depends upon all-hands to contribute your professional Tanker and Ontos crewman experience.

In the Command Chronologies Abstracts you will see the type of information we are compiling for the book.

Octobers' 1965 to 1970 - Marine Tanks & Ontos Action

This Breech Block - October 2014 - will provide you the chronology of the action in which the 1st, 3rd, and 5th Tanks and Antitanks were involved in during each October that they were in-country. For the complete command chronologies, please visit our Foundation website http://www.mcvthf.org where posted are the complete Tank and Antitank (Ontos) CCs. If you served in Tank or Ontos units during any month of October in any capacity or MOS and can recall some experiences that you would like others to read, jot them down and send them to us. Pictures with captions are of particular interest. Nearly all of the material you send us will find its way into one of our publications and "The Book".

  Also, you are encouraged to access the Foundation's Map Chronology that Pappy has created http://www.mcvthf.org. There you may find out exactly where your unit - battalion, company, often platoon, and occasionally section - was located at any given time. If you do not find your unit on one of Pappy's several on-line maps, let us know. Also is plotted the exact location and statistics of every Tanker and Ontos crewman who was killed in action.

Veterans Health Administration VA Access Audit

At the Department of Veterans Affairs (VA), our most important mission is to provide the high quality health care and benefits Veterans have earned and deserve - when and where they need it. In mid-April, the Secretary of Veterans Affairs directed the Veterans Health Administration (VHA) to complete a nation-wide Access Audit to ensure a full understanding of VA's policy among scheduling staff, identify any inappropriate scheduling practices used by employees regarding Veteran preferences for appointment dates, and review waiting list management.

VA is already taking corrective action to address issues resulting from the audit.

On Wednesday, May 21, VA launched the Accelerating Access to Care Initiative, a nation-wide program to ensure timely access to care. As directed by President Obama, VHA has identified Veterans across the system experiencing waits that do not meet Veterans expectations for timeliness. VA has begun contacting and scheduling all Veterans who are waiting for care in VA clinics or arranging for care in the community, while simultaneously addressing the underlying issues that impede Veterans' access.

In addition to the Access Audit process, VA also gathered additional data from each facility. This data includes: The number of appointments scheduled at each facility; the number of requested appointments that are on each facility's Electronic Wait List; The number of newly enrolled patients who have not yet been scheduled by facility; and Average Wait Times for Mental Health, Primary Care, and Specialty Care at each facility, for both new and established patients.

Twice Monthly Updates

VA is posting regular data updates showing progress on its efforts to accelerate access to quality health care for Veterans who have been waiting for appointments. These access data updates will be posted at the middle and end of each month at VA.gov. The facility-level data show the current status of VA's:

  • New Enrollee Appointment Request (NEAR) List: The total number of newly enrolled Veterans that have asked for an appointment during the enrollment process.  Out of an abundance of caution, VA reviewed the NEAR data from the past decade to identify any individual who: 
    • has enrolled for care since the inception of enrollment,
    • indicated they desired an appointment on the enrollment form, and
    • has not yet had a scheduled appointment in the VHA health care system.
  • Electronic Wait List (EWL) Count: The total number of all new patients (those who have not been seen in the specific clinic in the previous 24 months) for whom appointments cannot be scheduled in 90 days or less; and
  • Total Appointments Scheduled: Every appointment scheduled at the facility except surgery and medical procedures.

  • With the August 28 data release, pending and completed appointments are separated into different documents. Additional detail about completed appointments is now available as well.

  • The term "pending" has replaced the term "prospective," which predicts the availability of scheduled appointments in the future for new Veterans on a given date, and the term "completed" replaced the term "retrospective," which indicates when appointments actually occurred.

Initial data published for 5/15/14 contained accurate data except for the count and percent of appointments less than 30 days. The timeliness for new patients were erroneously computed using the "desired data" for the Veterans' appointment versus the date the appointment was initially created. The net effect of this error is that, as published, the 6/9/14 report represents 6.7% more appointments occurring within 30 days. This current posting of data corrects this error.
